我一直以为肾虚焦虑只见于中华文化辐射下的中老年男性，直到我遇到这位网诊病人：一位来自美国热带城市圣胡安的40多岁白人中年大叔。<\/p>\n

他的主诉有一大堆：高血压（148mmHg-190mmHg），慢性肾病（CKD），右胁下痛，腰背痛，全身皮肤瘙痒及多发深色红斑，性欲低，时有阳痿。消化不好，大便呈漂浮状，小便尿黄多泡沫（尿液检出红细胞及细菌），时有呼吸不畅，易疲劳，精神焦虑等。<\/p>\n

他说，他去看了当地中医说他是肝肾虚，给他吃六味地黄丸，吃了一段时间后他感觉消化好了一些，但是开始出现呼吸困难的情况，所以又不得不停药；而西医告诉他慢性肾病是无法恢复的，再加上全身瘙痒难耐，让他很焦虑，不得已一直在吃抗抑郁药物-百忧解。<\/p>\n

现在一说起来补肾，多想到的是六味地黄丸，但它确实多数情况下难当大任，是解决不了现代人复杂的肾虚问题的。<\/p>\n

就这个患者来讲，他的问题是比较复杂的，有肾气虚的因素（并非六味地黄丸的肾阴虚），有下焦湿热的问题，有肝气不舒的问题，还有血瘀的问题等。总体来说，患者的情况是虚实夹杂，这个时候纯用一个补虚的方，很容易一补就瘀堵加重。所以，用多几次之后，气机郁滞，开始有呼吸短气的问题出现了。<\/p>\n

我的治疗策略是前期以补肾气、疏肝理气及去湿热为主，后期以巩固肾气及消风祛瘀为主。<\/p>\n

目前还在治疗中，患者反馈血压非常稳定，以前血压波动造成的不适感完全消失，精神状态好转，小便问题明显改善，胁下疼痛消失，皮肤瘙痒及红斑大为好转，没有再出现呼吸不畅的问题。<\/p>\n

我多次劝他血压稳定后（让西医帮他）逐步停减他的西药试试，但他对中药调整后的现状很满意，似乎不愿意冒风险停减西药而造成血压波动。我当然能理解，毕竟看起来他有几个分公司在忙碌，身体处在比较稳定的状态是最好的现状。但是，长期服用降压药对肾脏造成的损伤是非常明确的。根本问题不解决，光焦虑有什么用呢？<\/p>\n
