Case Summary:
A 62-year-old Vietnamese male with a long history of chronic obstructive pulmonary disease (COPD) presented with symptoms of shortness of breath, fatigue, and inability to perform daily household tasks. He could walk no more than one kilometer per day. Additional complaints included chronic cough with excessive phlegm, unsatisfactory bowel movements, and urinary difficulties (frequent urination, urgency, and incomplete voiding). The patient exhibited a sallow, dark complexion, a thin body frame, and a hardened, distended area upon palpation under the heart region. His tongue appeared slightly red, moist with greasy coating, and sublingual veins were dark and distended. The pulse was floating, wiry, and strong.
Prescription:
Modified M**** Tang + Ban Xia Hou Po Tang + Gui Zhi Fu Ling Wan.
Results after 2 Weeks of Treatment:
The patient reported significant improvement in shortness of breath and fatigue, and he was able to perform household chores. Cough reduced by 70%, and he could walk more than two kilometers per day. Urinary frequency improved by 50%. Additionally, the patient mentioned a notable change: his body, which used to feel hot, now felt cooler after taking the prescribed herbs.
Follow-Up Observation:
Pulse diagnosis indicated that the disease mechanism persisted, so the patient was advised to continue treatment for another 3 weeks to consolidate the improvements.
Abdominal Examination:
Two weeks into treatment, an abdominal photograph was taken. Initially, the abdomen felt as hard as a rubber board upon palpation. By the follow-up visit, although still slightly firm, it had regained elasticity.
Update:
January 9, 2025 Follow-up feedback: Except for experiencing slight shortness of breath during bathing, everything else feels good.
