Case Study: Treatment of Suppurative Hidradenitis with Dan Zhi Xiao Yao San

A 28-year-old Western female presented with a complaint of right axillary suppurative hidradenitis, which had been infected for three weeks, with local redness, swelling, and suppuration. Upon examination, her tongue was pale with red dots at the tip, and her pulse was thin, rapid, and slippery. The diagnosis was liver and kidney yin deficiency, liver qi stagnation transforming into fire, combined with phlegm-dampness and blood stasis. The patient also reported poor sleep, irritability, and scanty menstrual flow.

The prescription included Dan-Zhi Xiao-Yao San combined with Gui-Zhi Fu-Ling Wan, with additional modifications. During the follow-up visit, the redness and swelling had significantly dissipated, and the patient reported feeling much more at peace and emotionally relaxed after taking the medication.

The patient’s complexion appeared slightly greenish, suggesting a liver imbalance. The acupoint “Ji Quan” (极泉), located in the axilla, was identified as the focal point of the condition, also corresponding to the heart meridian. This reflected the pattern of liver and kidney insufficiency, with liver qi stagnation transforming into heat, which then spread along the heart meridian.
